Ultrasonography

USG of WHOLE ABDOMEN MALE

LIVER             :        Liver is normal in size and echotexture. Intrahepatic biliary radicles are not dilated. No focal or solid mass lesion seen.  Portahepatis is normal.  Portal vein measures 10 mm in diameter.
GB                   :         GB is well distended with normal wall. No intraluminal calculus seen. Liver / GB demarcation is clear.
CBD                :         CBD is normal in diameter (4 mm). No calculus is seen in CBD

PANCREAS    :         Pancreas shows normal echotexture. MPD is not dilated. No peripancreatic fluid is seen

SPLEEN          :         Spleen is normal in size and normal in echotexture.

KIDNEYS       :         Both kidneys are normal in size, shape, position with normal cortical echotexture. CMD is maintained bilaterally. No calculus or mass or hydronephrosis is seen in both kidneys. RK measures 83 mm and LK measures 93 mm.
  
  UB                  :         Urinary bladder is well distended. No mass or calculus seen. UB wall is normal in thickness.

PROSTATE       :        Normal in size and echotexture. Estimated weight ~14 gms

LIVER             :        Liver is normal in size and normal in echotexture. Intrahepatic bil
GB                   :         GB is well distended with normal wall. No intraluminal calculus seen. Liver / GB demarcation is clear.

CBD                :         CBD is normal in diameter (4 mm). No calculus is seen in CBD

OVARIES     :          Both the ovaries are normal in size and echotexture. No focal solid or
                                   cystic lesion is seen in the ovaries
No ascites seen.
No adenopathy seen.
Impression     :  * No significant abnormality
HIGH  RESOLUTION USG OF  NECK :
- Both lobes of thyroid gland are normal in size and  echotexture.
  No focal solid mass or calcification seen. No focal areas of
  hypoechogenicity is seen.
  Right lobes measures  37 x 11  mm
  Left lobe measures   30 x 11 mm

- Isthmus thickness is normal.

- Vascularity of thyroid gland is normal. 

- Bilateral parotid and submandibular glands are normal.
USG OF PREGNANCY

FETUS : Two live fetuses are  seen

Fetus A in breech presentation
BPD = 82 mm., FL = 89 mm, HC = 294 mm, AC = 249 mm
FHR : 132 bpm
GA by USG   : 31 weeks 3 days
EFW : 1567 kg ± 229 gms.
Cord around the neck        : Not seen 

Fetus B in cephalic presentation
BPD = 85 mm, FL = 65 mm, HC = 305 mm, AC= 270 mm
FHR = 136 bpm
GA by USG   : 33 Weeks 2 days
EFW : 1990 kg ± 291 gms.
Cord around the neck        : Not seen 
                                               
PLACENTA   : Placenta is seen posteriorly and towards left side body. It is not low lying. Grade II.

LIQUOR VOLUME : Liquor volume is normal.  Largest Liquor pocket is 36 mm

DOPPLER : No reversal or absence of diastolic flow in umbilical arteries of both the fetuses
IMPRESSION :
* Twin live intrauterine pregnancy
* Twin A in breech presentation of 31 weeks 3 days maturity ( EFW ~ 1567 gms)
* Twin B in cephalic presentation of 33 weeks 2 days maturity ( EFW ~ 1990 gms)
* No reversal or absence of diastolic flow in umbilical arteries of both the fetuses
HIGH RESOLUTION USG OF SCROTUM :
TESTIS.                    : Both testicles are normal in size, shape and echotexture.
                                    No focal hypoechogenicity or calcification seen.
                                    Right testis measure: 46 x 23  mm.
                                    Left testis measures:  40 x 20 mm.

EPIDIDYMIS             :Bilateral epididymis are normal in outline and echogenicity.
                                    No cystic or solid mass seen.
                                    Right measures : 16 x 8.6 mm
                                   Left measures : 10 x 6.4 mm

FUILD POCKET:     Bilateral fluid pockets are normal.
Bilateral cord veins are normal in caliber. No varicosities seen.
Testicular vascularity is normal.
IMPRESSION  :         Ù NORMAL STUDY.
Ù no adenopathy seen.

LK measures 106 x 51 mm.

Renal ostia & proximal parts of bilateral renal arteries are obscured by overlying bowel gas
Main renal arteries are seen at hilar region
No abnormal narrowing seen at this level
PSV less than 100 cm/s at this level

Intrarenal vessels in both kidneys show normal flow pattern.
Resistivity index of intrarenal vessels are normal in both kidneys.
Acceleration time is normal in intrarenal vessels of both kidneys.

- Parvus tardus flow pattern in intrarenal vessels of both kidneys with increased acceleration time------Indirect evidence of significant bilateral renal artery stenosis.
- Main renal arteries are not completely visible due to overlying bowel gas.

Adv : CT angiography of renal arteries to rule out stenosis at renal artery ostium

USG LOWER ABDOMEN

KIDNEYS       :         Both kidneys are normal in size, shape, position with normal cortical echotexture. CMD is maintained bilaterally. No calculus or mass or hydronephrosis is seen in both kidneys. RK measures 97 mm and LK measures 100 mm.

  UB                  :         Urinary bladder is well distended. No mass or calculus seen. UB wall is normal in thickness

UTERUS    :             Uterus is normal in size and echotexture. No focal myometrial lesion is seen. Endometrium is normal. Uterus measures mm. Cervix is normal

OVARIES     :          Both the ovaries are normal in size and echotexture. No focal solid or
                                   cystic lesion is seen in the ovaries

No ascites seen.
No adenopathy seen.
                                                                             USG OF KUB MALE

KIDNEYS       :       Both kidneys are normal in size, shape, position and echotexture. No calculus, cyst, or hydronephrosis is seen. Ureters are not dilated. RK measures 102 mm and LK measures 103 mm.

  UB                    :       Urinary bladder is well distended. No mass or calculus seen.

PROSTATE    :       Prostate gland is normal in size and echotexture. No focal mass or calcification seen. Prostate size — 39x24x28 mm & weight 14 gms.

S.VESICLE     :       Seminal vesicles are normal in outline and echogenicity. 

No ascites seen.
No adenopathy seen

IMPRESSION:         * USG features are within normal limits.
                                                                             USG OF KUB FEMALE

KIDNEYS       :       Both kidneys are normal in size, shape, position and echotexture. No calculus, cyst, or hydronephrosis is seen. Ureters are not dilated. RK measures 102 mm and LK measures 107 mm.

  UB                    :       Urinary bladder is normally distended. No mass or calculus seen.

UTERUS    :             Uterus is normal in size and echotexture. No focal myometrial lesion is seen. Endometrium is normal. Uterus measures mm. Cervix is normal

OVARIES     :          Both the ovaries are normal in size and echotexture. No focal solid or cystic lesion is seen in the ovaries

No ascites seen.
No adenopathy seen.
IMPRESSION:         * USG features are within normal limits

Early Preg

UTERUS : Uterus is bulky with normal myometrial echotexture. No myoma seen.

A well defined rounded fundal implantation intra uterine gestational sac is seen
with a viable embryo.

An approx. mm sized rounded yolk sac also seen within the gestation sac.

The USG estimated gestational age is  weeks days.
(MSD-    cm,  wks  days; CRL-  cm,  wks  days).

The USG estimated EDD is .

Fetal heart rate measures  BPM at the time of scan and regular.

No evidence of any subchorionic haemorrhage seen.

Cervix is normal in length. Os is closed.

OVARIES  : Bilateral ovaries are normal in size and echotexture.
No free fluid seen in cul-de-sac.

IMPRESSION :-
- WKS DAYS GESTATIONAL AGE SINGLE LIVE INTRA UTERINE PREGNANCY.
   STUDY OF right LOWER LIMB 

Colour duplex Doppler study of both venous and arterial system of right lower
limb was evaluated using high frequency 7.5 MHz probe

Findings  : 

Arterial system of right lower limb is normal in course and caliber. Intimomedial calcification is seen at places in right lower limb vessels. No stenosis / dilatation is noted. Spectral study reveals normal triphasic flow in the arteries of both  lower limb with no area of turbulent flow.
Gradual reduction of peak velocity is seen distally.

Bilateral Sapheno-femoral and sapehno popliteal valves, great saphenous veins (GSV), common femoral, superficial femoral and popliteal veins are normal in caliber, compressible and shows normal respiratory variation. Post-compression augmentation is present. No thrombus is seen. No varicosities or perforator incompetence is noted at present.
IMPRESSION  :  
- Intimomedial calcification in right lower limb vessels likely due to atherosclerotic changes.
- Normal triphasic flow in right lower limb arteries.
- No deep vein thrombus is seen.
                                                            USG WHOLE ABDOMEN ( SCREENING)


LIVER             :        Liver is normal in size and echotexture. Intrahepatic biliary radicles are not dilated. No focal or solid mass lesion seen.  Portahepatis is normal.  Portal vein measures 9 mm in diameter.
 
GB                   :         GB is well distended with normal wall. No intraluminal calculus seen. Liver / GB demarcation is clear.

CBD                :         Visualized CBD is normal in diameter (4 mm). No calculus is seen in visualized part

PANCREAS    :         Pancreas is normal in size, shape, position and echotexture. MPD is not dilated. No peripancreatic fluid is seen

SPLEEN          :         Spleen is normal in size and echotexture.

KIDNEYS       :         Both kidneys are normal in size, shape, position and echotexture. No calculus or mass or hydronephrosis is seen in both kidneys.

Bright intraabdominal fat is seen in left lower quadrant of abdomen with overlying probe tenderness


Impression: 
Bright intra-abdominal fat in left lower quadrant of abdomen with overlying probe tenderness---likely due to Epiploic appendagitis/ Acute omental infarct

CAROTID DOPPLER STUDY

Common carotid arteries

Intimomedial thickening is seen in bilateral common carotid arteries.
Right CCA IMT = 1.1 mm
Left CCA IMT = 1.1 mm
Normal filling of bilateral common carotid arteries with colour signals is seen.
Flow pattern in bilateral common carotid arteries are normal with normal peak velocities.
No plaque seen in bilateral common carotid arteries.

ICA & ECA

Visualized bilateral extracranial ICA show normal flow pattern ( normal low resistance flow pattern). No plaque is seen in arterial wall in both sides. No dilatation or narrowing of the artery is seen bilaterally.

Visualized bilateral ECA show normal flow pattern ( high resistance triphasic flow pattern). No plaque is seen in arterial wall in both sides. No dilatation or narrowing of the artery is seen bilaterally


Vertebral arteries
Bilateral vertebral arteries show low resistant flow pattern and flow directed towards brain. No steal phenomenon is seen bilaterally.

IJV : No thrombus is seen in bilateral internal jugular veins.


Impression:
- Intimomedial thickening in bilateral CCA --- likely due to atherosclerotic changes.
- No plaque/ thrombus in bilateral extracranial ICA, bilateral ECA and bilateral CCA.
- Vertebral arteries show flow directed towards brain. No steal phenomenon is seen in both sides.
